Some of the items might have empty slots for gems.
In order to apply gem on item, you have drag&drop gem on desired item.
Gems can be dropped by hostile mobs in any world or traded by some NPC. Also needs to be noted that, stats of dropped gems are based on mob levels, meaning, higher level mobs drop gems with better stats.
